Three ways to send an order. Pick the one your office already uses.

## Order in the physician portal

Place the order, track the study and open images and reports as soon as they are signed.

## Fax the order

One fax number for all five centers. Include the diagnosis and ICD code, the study, the contrast preference, and a cell number for STAT or critical findings. (915) 400-0427

## Use our order form

Every study we perform, by center, on one page, with the patient preparation sheet on the back. Print it, mark the study, then fax or email it.

## What a complete order needs

Incomplete orders delay the patient's exam.

- Referring provider, signature and date
- Diagnosis and ICD code
- The study, the side, and contrast: with, without, both, or radiologist's protocol
- Patient name, date of birth and phone number
- Insurance and policy or group number
- Report preference: routine, or urgent within 24 hours
- A cell number for STAT or critical findings

## Prior authorization

MRI, CT, PET/CT and nuclear medicine may need prior authorization from the patient's plan. Send it with the order when you have it.

## Reports and images

Routine or urgent within 24 hours, your choice on the order. Reports go to the portal and by fax. Images and reports for your patients are also available through PocketHealth, without a CD.

## Patients without coverage

Private-pay rates are available for every exam.

## Your physician liaison

Five liaisons cover El Paso practices. They set up portal access, bring order forms, and sort out scheduling and results questions.
